CONTINUING EDUCATION PROCESS

Michigan HomeCare & Hospice Association is approved as a provider of nursing continuing professional development by Pennsylvania State Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation.

In order to receive CE credit, you MUST complete the following steps while attending the convention. If you do not follow these steps, you will not receive credit for the offering in question:

1. Obtain your labels from the CE table by the registration desk during the hours indicated in the program.

2. Within the first 15 minutes of the offering, place one of your labels on the WHITE sign-in sheet for nurses or the PINK sign-in sheet for Social Work provided during each CE offerings. It is YOUR responsibility to ensure that your label and signature are on the sign-in sheet within the first 15 minutes of the CE offering. All sign-in sheets will be removed for processing 15 minutes into the program

3. Sign the sheet in the space provided next to your label.

4. At the conclusion of the session, place another label on the WHITE (nurse) or PINK (social work) sign-out sheet in the back of the room and sign your name next to your label.

5. At the conclusion of the session use the QR Code provided in the session to complete your evaluation of the session.

6. Once you have sign in, then signed out and completed your evaluation, your certificate for that session will be released.

PLEASE NOTE: Contact hours will NOT be awarded if we don’t have a label on the sign in sheet, Sign out sheet and a completed on-line evaluation at the conclusion of the conference for each session. NO EXCEPTIONS!
