The Michigan HomeCare and Hospice Association is proud to serve as the State Hospice Organization lead for the Michigan Hospice-Veteran Partnership (HVP). The Hospice-Veteran Partnership is a national program of the U.S. Department of Veterans Affairs (VA) Hospice and Palliative Care Initiative that brings together VA medical centers, community hospices, state hospice organizations, and Veteran-serving organizations to ensure every Veteran has access to excellent end-of-life care and that their families receive the support they deserve.

Through the Michigan HVP, MHHA works alongside VA facilities across the state and our hospice members to strengthen relationships between community hospices and the VA, improve Veterans' access to hospice and palliative care, support smooth transitions across care settings, and provide education on the VA health care system, Veterans' benefits, and Veteran-specific clinical needs.
