Motoring Through Change…Join the Ride…the theme for this year’s Michigan Foundation for Home Care 2015 Annual Conference and Exposition.

Every one of us is experiencing change in some capacity or another. It’s all around us. In order for all of to survive in this world of change, we must learn to make good business decisions, keep up on regulatory changes, and have a plan on how to survive and thrive as we motor through change.

Mail completed registration form with payment to Michigan Association for Home Care, 2140 University Park Drive, Suite 220, Okemos, Michigan  48864.  Make checks payable to Michigan Association for Home Care.   No telephone registrations will be accepted.  Fax registrations will be accepted ONLY for payment by VISA, MasterCard, American Express or Discover.  Confirmations are guaranteed to be sent to those whose registrations are postmarked by Friday, May 1, 2015.  Early bird registration discount rates apply to registrations postmarked by Friday, April 10, 2015.  Agencies will be billed for the difference if early-bird fees are postmarked after Friday, April 10, 2015.  The Late rate is for registrations postmarked April 11, 2015 or after. 

Registration sharing is strictly prohibited.  Each person must be registered.


One-Day Registration:

One-day registration is available.  One-day registration includes:  educational programming, handouts, continental breakfast, breaks and any planned meal activities or receptions scheduled for your day of attendance.


Cancellation Policy:

Registration fees, less a $50.00 processing charge are refundable by submitting your request in writing to Michigan Home Care (fax:  517/349-8090) on or before Monday, April 13, 2015.  No refunds will be made, under any circumstances, after April 13th.  Substitutions are permitted by faxing your request to Michigan Home Care, 517/349-8090.


Nursing Contact Hours:

The Michigan Association for Home Care (OH-290, 3/1/2016) is an approved provider of continuing nursing education by the Ohio Nurses Association (OBN-001-91), an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation.


Commercial Support:

There is no commercial support for  the educational sessions.



There are sponsors for  some of the sessions and is indicated under each session through the agenda.


Conflict of Interest:

The planners and faculty have declared no conflict of interest. 


Criteria for Successful Completion:

In order to receive contact hours you must sign in with your barcode labels (obtained at the CE table by registration) at the start of each session.  At the conclusion of each session, you must complete your evaluation and put one of your barcode labels on the completed evaluation and turn it in as you leave the room.


Social Work CEs:  Applied for to the Michigan SW CE Collaborative.

For your comfort, please dress casually for the conference.


Meeting Rooms:

Individual response to temperature differs greatly.  It is best to layer clothes…dress for a warm room, but bring a sweater or jacket.

Golf will be available to attendees on Thursday, May 14, 2015 on the  Wolverine.  Cost is $40 a person for 9 holes.  4:00 p.m. start .  Call 231/534-6470 to sign up.  Credit Card will be asked for.  48 hour cancellation.  Deadline for pre-registration is May 7th.


Silent Auction: 

Plan to attend and participate in the Silent Auction benefit the Michigan Foundation for Home Care .  Shopping right at your door step!  The  Annual  Silent Auction will be set up in the Exhibit Hall starting on May 13, 2015 from 5:30 p.m. to 7:00 p.m. and will remain open throughout the conference during scheduled exhibit times.  The bids will close at 12:00 noon on Thursday, May 14, 2015.  Proceeds will help support the

Michigan Foundation for Home Care Education scholarships and programming.   If you are interested in donating something to the auction, complete the enclosed donation form.

T. Brian Callister

Opening Session:  The Continuum and the Post Acute Balancing Act

T. Brian Callister, M.D. is a Board Certified Internal Medicine specialist and Hospitalist who is nationally recognized as an expert in both acute care medicine and the post-acute continuum. He is actively engaged in healthcare policy debates, end-of-life issues, and spirituality in health. His diverse blend of technical expertise, medical experience, humor, and motivating spirituality are attractive to a wide variety of audiences both inside and outside of the medical field.

Dr. Callister is the National Medical Director for LifeCare Hospitals, one of the largest Long Term Acute Care (LTAC) hospital systems in the country, and has directed its medical staff leadership for fifteen years.  As the Chairman of the Clinical Committee of the Acute Long Term Hospital Association, he led the effort to develop the first LTAC industry outcome benchmark study and has been a leader in acute criteria development for both the American Hospital Association and The Joint Commission.

Adam Townshend is the Assistant United States Attorney with the U.S. Department of Justice out of Grand Rapids.   As a government attorney with significant experience handling civil and criminal health care fraud cases, he has expertise in and a particular sensitivity to, the fraud schemes that are increasingly pervasive in the health care industry.

Richard Kroll Jr. has an extensive and varied background in business, education, ministry, and leadership development.  He served in the Medical Corps of the United States Navy and specially trained with the United States Marines.  Earlier in his career he was a sales trainer and motivational speaker.  Not too long after starting his career, he served in upper management in the advertising and publishing industry. In an effort to express his faith in his vocation, he became a teacher and eventually a principal, educational consultant, and executive director; starting and improving schools domestically and internationally.  Richard has served as a senior pastor of several churches and as a missionary in the United States, Africa, Turkey, India, Mexico, Bahamas, Haiti and Costa Rica.

For more than a decade Sophia has worked with business leaders in the health care industry to reduce their risk.  She understands the challenges facing the industry from regulations to health care reform and increase of minimum wage all having a negative impact on cash flow.  She works with companies to examine all areas of risk and implementing a  proactive prevention strategy to reduce the total cost of risk associated with their business   This approach has proven to have a positive impact on  a business’s employees, clients and profits.

John brings more than 25 years experience developing leaders across a wide array of diverse industries.  Before becoming a Master Certified Business Advisor and a Certified Executive Coach, he was responsible for leadership development as an executive at three Fortune 500 Companies:  Ascension Health, Oakwood Healthcare and Ford Motor Company.  Served as adjunct professor at two colleges and has been instructor of new Executive Coaches Worldwide.
