Registration:

Mail completed registration form with payment to MHHA, 2140 University Park Drive, Suite 220, Okemos, Michigan  48864.  Make checks payable to MHHA.   No telephone registrations will be accepted.  Fax registrations will be accepted ONLY for payment by VISA, MasterCard, American Express or Discover.  Confirmations are guaranteed to be sent to those whose registrations are postmarked by Friday, May 10, 2019.  Early bird registration discount rates apply to registrations postmarked by Friday, March 31, 2021.  Agencies will be billed for the difference if early-bird fees are postmarked after Friday, March 31, 2021  The Late rate is for registrations postmarked March 31, 2021 or after.  Registration sharing is strictly prohibited.  Each person must be registered. 

Cancellation Policy:

Registration fees, less a $50.00 processing charge are refundable by submitting your request in writing to MHHA (fax:  517/349-8090) on or before Friday, April 2, 2021.   No refunds will be made, under any circumstances, after April 2nd. Substitutions are permitted by faxing your request to MHHA, 517/349-8090.

Nursing Contact Hours:

The Michigan HomeCare & Hospice Association is approved  as a provider of  nursing continuing professional development by the Ohio Nurses Association an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation. (OBN-001-91) .

Commercial Support:

There is no commercial support for  the educational sessions.

Sponsorship:

There are sponsors for some of the sessions and they are indicated under each session throughout the agenda.

Conflict of Interest:

The planners and faculty have declared no conflict of interest. 

Criteria for Successful Completion:

In order to receive contact hours you must stay for the entire session and complete an evaluation for each session.  The virtual platform will provide us with attendance from each session when people sign in to that session and sign out.   At the conclusion of each session,  you must complete the evaluation. 

Social Work CEs:  Applied for to the Michigan SW CE Collaborative.

Physical Therapy CEs:  Applied for to the Michigan Physical Therapy Association

Nurse Home Administrators CEs:  Applied for to the Michigan Department of Licensing and Regulatory Affairs

Consent to Use Photographic Images

Registration and attendance at, or participation in, MHHA meetings and other activities constitutes an agreement by the registrant to the use and distribution of the registrant or attendees’ image photographs of such events and activities by MHHA.
